The Virtual Lung Screening Trial (VLST) is a large-scale in silico study designed to emulate key aspects of the National Lung Screening Trial (NLST) using computational patient models, scanner-specific imaging simulation, and AI-based virtual readers. The resource includes virtual patient cohorts derived from XCAT phantoms with simulated pulmonary nodules of varying morphology, paired simulated CT and chest radiography (CXR) acquisitions, and corresponding AI-reader outputs for lesion detection and classification. VLST provides a controlled framework for investigating imaging system performance, reader behavior, and trial design in lung cancer screening, enabling reproducible evaluation of imaging technologies without the cost, time, or patient risk associated with conventional clinical trials.
